    It means she's really scared while risking her life playing Russian Roulette. When writing the song, she didn't mean playign the game literally but something just as scary. (I'm not sure what it is.) She is also saying that she can't quit what she has

    well i think shes using the term "russian roulette" as a sort of metaphore to say shes taking a chance on getting really hurt in a relatioship, but she felt it was something she had to do. and it was scary, but she was at a point of no return.
